Stefany Rankin's first career goal gave the visitors a 1-0 lead with five minutes left in the first.
Brooke Einarson assisted on Rankin's goal, which was scored on a power play.
Regina held onto its 1-0 lead until late in the third period, when Shelby Ballendine tied up the game with a shorthanded marker.
Janzen's goal, which was assisted by Charlette Hainer, gave the Pronghorns their second win of the season – both of which have come over the Cougars.
The Cougars outshot Lethbridge by a 38-25 margin during the contest, but goalie Mackenzie Rizos made 37 saves to secure the win for the Pronghorns.
Lisa Urban made 23 saves in the overtime loss for the Cougars.
The Cougars move to 3-13-1 on the season, while Lethbridge improves to 2-14-1.
The two teams will meet again on Saturday night in Lethbridge.
