Medical Forms & Information
First-Year Athletes - [Rookie Medical Form]
This includes all athletes new to the University of Regina Athletics Program (includes transfer students) as well as any athlete who has taken one or more seasons away off of their varsity sport. Athletes filling out this form will need to complete a medical examination by a physician and have the physician fill out the Medical Examination portion of the form.  The athlete is responsible for any costs incurred for the completion of this form. The form must be completed in full, signed, and submitted to the Athletic Therapy department prior to participation in any practices or training camps.

Returning Athletes - [Health Reassessment Form]
This includes athletes who have competed for the University of Regina over the past competitive season and have a Rookie medical form on file. All returning athletes are required to submit a completed University of Regina Annual Health Reassessment Form. Upon review of this form by the head therapist and/or the physician, athletes may be required to be reviewed by a physician to gain medical clearance for participation. The University of Regina Rams Football Club require an annual medical for new and returning athletes.

Participation in University athletics may result in physical injury and/or in the aggravation, deterioration and/or re-injury of any pre-existing medical condition(s) during and after one's involvement with the University of Regina. It is important that athletes fully understand and assume the risks of participating in University athletics. The University of Regina and University of Regina Athletics will not pay for expenses that are normally covered under any other applicable insurance or government plan. Athletes participating with a University of Regina Athletics team must have a valid Provincial Health Care/Hospitalization Number AND have medical coverage through a private / 3rd party insurance policy. Athletes who do not have the above health care insurances will be denied participation. University of Regina full time students are required to carry the URSU Student Health Care Plan, or provide proof of alternate third party plan to be eligible to opt out of the University plan.

Athletic Scholarships
The Student Awards Management System is used to search for and apply for undergraduate awards at the University of Regina.

It is a goal of the University of Regina to assist in lessening the financial burden of our student-athletes within the policies laid out by U SPORTS.

At a minimum, the following requirements must be met to be eligible for athletic scholarships:
• Entering students require a minimum 80% admission average
• Returning students (including transfers) require a minimum 65% on all registered courses within the last academic year (September - August) and completion of 18 credit hours (or equivalent)
• Student-athletes must be listed on the team's eligibility certificate
• The maximum a student-athlete can receive in athletic scholarships is the value of their tuition and mandatory fees

In addition to athletic scholarships, the University of Regina offers hundreds of other financial awards to eligible students. While the amount of athletic scholarships one can receive is limited to the value of tuition and mandatory fees paid, a student can receive academic scholarships over and above what they receive in athletic scholarships.
